We are seeking a motivated and patient-focused Clinical Pharmacist to join our friendly and forward-thinking GP practice at Storrsdale Medical Centre. This is an exciting opportunity to play a key role in improving patient outcomes, supporting medicines optimisation, and working collaboratively within a multidisciplinary primary care team.
You will work closely with GPs, nurses, and administrative teams to deliver high-quality, safe, and effective prescribing practices while supporting patients with long‑term conditions and complex medication needs.
Main duties of the job
- Undertake structured medication reviews and medicines reconciliation
- Support safe, cost‑effective prescribing in line with local and national guidance
- Manage and monitor long‑term conditions (e.g. hypertension, diabetes, asthma)
- Respond to medication queries from patients and staff
- Work with the practice team to improve prescribing systems and processes
- Deliver medicines optimisation and safety initiatives
- Support repeat prescribing systems and reduce prescribing errors
- Provide patient‑facing clinics and telephone consultations where appropriate
- Contribute to QOF, IIF, and enhanced services delivery
- Liaise with community pharmacies, PCN colleagues, and secondary care

Key Responsibilities
- Conduct clinical medication reviews for patients with complex needs
- Manage prescribing for long‑term conditions
- Identify and manage medication‑related risks
- Implement deprescribing where appropriate
- Provide patient education and support adherence
Prescribing & Medicines Optimisation
- Ensure prescribing follows NICE guidelines and local formularies
- Monitor high‑risk drugs and undertake safety audits
- Support cost‑effective prescribing initiatives
- Manage repeat prescription processes
Patient‑Facing Services
- Run clinics (face‑to‑face/telephone/online)
- Address medication queries
- Provide advice on minor ailments and medication usage
Quality Improvement & Governance
- Contribute to audits and service improvement projects
- Support implementation of prescribing safety alerts
- Assist in achieving QOF and enhanced service targets
Collaboration
- Work with GPs, nurses, practice staff, and PCN pharmacists
- Liaise with community pharmacies and hospital teams
- Participate in MDT meetings where required
Training & Development
- Support and mentor practice staff on medicines‑related issues
- Maintain CPD and professional registration requirements

Qualifications
- Masters degree in Pharmacy (MPharm or equivalent)
- Registered with the General Pharmaceutical Council (GPhC)
- Experience working as a pharmacist in primary care, community, or hospital setting
- Knowledge of medicines management and optimisation
- Experience of working within a multidisciplinary team
