Integrated Care Clinician (Paramedic) – Cardiff
Job Summary
The Integrated Care Clinician will conduct telephone triage assessments of patients, advising on the most appropriate intervention in accordance with Trust policy and national guidelines. The role is autonomous, requires strong clinical assessment skills and the ability to make independent decisions within agreed boundaries.
Responsibilities
* Conduct telephone triage assessments and advise callers on the most appropriate intervention.
* Apply best clinical practice, adhering to Trust policies, protocols and guidelines.
* Make independent clinical decisions and manage patient care pathways.
* Collaborate with multi‑disciplinary teams including GP’s, pharmacists, mental health practitioners and other triage staff.
* Maintain accurate and up‑to‑date patient records.
* Support continuous improvement in triage procedures.
Person Specification
Essential
* Registered with the Nursing and Midwifery Council (adult or child) or the Health and Care Professions Council – registered paramedic with post‑registration experience in front‑line or telephone triage services.
* Evidence of ongoing professional development and update training.
* Flexible approach to work, including rotating shifts, nights and weekends.
* Strong listening and communication skills.
* Team‑working mindset with a collaborative style.
* Independent initiative and decision‑making capability.
* Sound clinical judgement and up‑to‑date clinical knowledge.
* Computer literate and committed to personal development.
* Good self‑management, time‑keeping and ability to work without direct supervision.

* Ability to speak Welsh.

Working Hours & Conditions
Permanent position – full‑time, part‑time or flexible working. Band 6 salary £40,559 to £48,841 per annum (pro rata). Working pattern includes rotating shifts, nights and weekends.
