Do you thrive in a fast paced, multi-agency environment?If the answer is “Yes” then this might be just the role for YOU!The South Transfer of Care Team is seeking a full‑time Senior Social Worker, based primarily at our Trumpington office near Addenbrooke’s Hospital in Cambridge. While this is a countywide service, the South team mainly supports discharges from Addenbrooke’s hospital, as well as Papworth Hospital, West Suffolk Hospital in Bury St Edmunds, Newmarket Hospital (Rosemary Ward), the Cambridge Rehabilitation Unit, and the Princess of Wales Rehabilitation Unit in Ely. We also assist with discharges from care homes following health‑led discharges. In addition, our team provides ongoing support after discharge by carrying out six‑week reviews to ensure individuals’ needs continue to be met in the community.Transfer of Care Teams work within the following hours: Monday to Thursday: 9:00am – 5:00pmFriday: 9:00am – 4:30pmOur team works in a hybrid model, combining time at the office with the flexibility of working from home.We are committed to supporting flexible working opportunities and staff can also benefit from salary enhancements depending on their chosen working pattern: 7% enhancement – for working 1 weekend day per month and 1 Bank Holiday per year8% enhancement – for working 1 weekend day per month and 2 Bank Holidays per year10% enhancement – for working 1 weekend day per month and 3 Bank Holidays per year15% enhancement – for working 2 weekend days per month and 3 Bank Holidays per yearIn addition, staff who work one or two weekend days each month will receive a day(s) off during the week in recognition of their contribution. We would particularly welcome applicants who already hold, or are working towards, a Practice Educator, Best Interests Assessor, or Approved Mental Health Professional qualification. You will have worked with a complex caseload and have excellent knowledge of current legislation which will include the Care Act 2014, Mental Capacity Act 2005 and the associated statutory guidance. You will have proven knowledge and skills for Safeguarding Adults.You will be a person-centred practitioner with excellent communication skills and have the ability to work within a multidisciplinary setting.
