We are looking for an expert by experience that has lived experience of being a carer of someone who has mental health issues. The Peer Support Carer's Lead works collaboratively and aims to assist in improving the wellbeing of families and carers of people who are admitted to the Crisis Resolution and Home Treatment Team by supporting carers directly, either via 1:1 or groups, developing and maintaining relationships with the service provider teams, providing carers with updates of their relatives care and treatment, contributing to service development and research.
Responsibilities
* Initiate, establish and maintain relationships with families and carers in a variety of settings, while developing trust and rapport.
* Act as a coach and mentor to families and carers by providing information about support and services available for them, ensuring they have carer communication plans where required.
* Work collaboratively with inpatient/CRHT/ATS to support carers via 1:1 support or group settings.
* Work within The Triangle of Care standards and undertake self assessments and action plans.
Qualifications
* Have lived experience of being a carer of someone with mental health issues.
* Can work independently and as part of a multidisciplinary team with confidence.
* Confident working closely with colleagues and service users alike.
* Previous NHS experience in a similar role (required for higher pay band). Successful candidates are normally appointed to the bottom of the pay scale unless previous NHS experience in a similar role can be evidenced.
* Completed relevant safeguarding training. SPFT takes its responsibilities for safeguarding children, young people and adults at risk very seriously. All staff and bank workers must share this commitment and complete the relevant safeguarding training for their role.
* Understanding of NHS and voluntary sector services to support carers navigating them.
* Ability to signpost to local carer support groups and provide information on rights and expectations.
