You will be fully supported to gain new skills and experience needed to deliver high-quality nursing care to patients in their own home or residential setting. As a community nurse, we will develop your wound care, palliative care, and long-term conditions knowledge and skills; supporting you to become an expert in assessment and care planning to deliver quality care that is caring and compassionate. * Keeping the patient at the centre of care, ensuring patient and carer participation in decision-making
* Providing a high standard of care
* Providing a high standard of nursing intervention within a patient's own home, with access to Specialist and Advanced Practitioners when required
* Working as part of a 24/7 service and multidisciplinary team
* Traveling independently around the county meeting strict time deadlines. You will need a full UK driving licence and use of a vehicle. Public transport cannot be used for this role.
We are looking for people who:
* Have experience working in the Community
* Are educated to RGN level 2
* Are able to demonstrate a commitment to high-quality care and service provision
* Have the ability to work flexibly and as part of a team
* Have the ability to assess and formulate care plans
* Are able to teach and support student Nurses and Healthcare Assistants
* Have enhanced communication and interpersonal skills
