Are you a qualified Pharmacy Technician who is passionate about making a difference to patient care in the community? Do you want to be part of a pioneering service supporting patients in their own homes?
If this sounds like you, then apply to join the innovative Intermediate Care (Hospital@Home) team at Cwm Taf Morgannwg University Health Board. This is a great opportunity to work in a dynamic, multi-disciplinary environment where your expertise will improve patient outcomes and support their independence at home. If you're looking for a role that blends clinical knowledge with community care, this could be the perfect fit.
Main duties of the job
The successful candidate will provide medicines management support and education to patients in their homes, including after their discharge from hospital. By assessing patients’ ability to manage their medication and supporting them with tailored care plans, the post holder will help maintain people safe and independent in their own homes. Other duties will include liaising with GPs, community pharmacies, and care providers to ensure safe and effective medication use. By collaborating with social care partners, including the delivery of medication training to care workers, the successful candidate will support the integration of health and social care. Working independently but within a supportive team, the role will utilise clinical skills and evidence-based approaches to improve patient outcomes.
Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn’t mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates. This isn’t ‘fluency’, just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health.
