Main duties of the job
We are seeking skilled and enthusiastic nurse with chronic disease experience to join our nursing team. The position is available for an immediate start. If you have the required general practice experience, we would be delighted to receive your application.

BlackthornHealth Centre and Hedge End Medical Centre have merged and are now known asHamble Valley Health.
Ourlist size is over 37000, and our patients are mainly concentrated in Hamble,Bursledon, Hedge End and Botley. Our aim is to provide high quality, accessiblemedical care to our patients, whilst maintaining a supportive and pleasantworking environment for clinicians and staff.
Weare part of Hampshire and Isle of Wight ICB, and offer General Medical Servicesto our patients.
Weare a training practice, and take registrar doctors and also teach medicalstudents from Southampton School of Medicine.
The practice has eightPartners. The current Partnership team is Dr Tomson, Dr Harris, Dr Cropley, DrGraham, Dr Sonpal, Dr Shahsavanspour, Dr Yates and Dr McKenzie.
We employ fourteen salariedGPs, five Advanced Nurse Practitioners, a Nursing team of fifteen, and areception/admin team of seventy.
The practice uses Emis as aclinical system. We are paper light, so scan all incoming correspondence ontopatients medical records.
Job responsibilities
* Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs
* Immunisations
* Chronic diseases
* Chaperoning and assisting patients where appropriate who are being examined by another clinician
* Requesting pathology tests, for example urine culture, swabs
* Following agreed clinical protocols with referral to senior nurses or GPs as appropriate
* Maintain general tidiness and cleanliness of nurses and treatment rooms
* Cytology
* ECGs
* Asthma/COPD
* Venepuncture
* Routine immunisations/Childhood immunisations
* Diabetes
* Cardiovascular disease
* Contraception
Person Specification
Qualifications
* Current NMC Registration.
* Post registration Nursing experience.
Experience
* Insight and understanding of current issues in nursing and the wider NHS.
* IT literate.
* Chronic disease management experience (COPD, Asthma, Diabetes)
* Understanding in health, safety, and infection control issues.
* Familiarity with the Emis clinical system.
* Mentorship qualification/desire to work towards this.
* Demonstrate an understanding of audit.
