Overview
Health and Therapy Assistant – Home First role at Sussex Community NHS Foundation Trust. Our multidisciplinary teams provide a 7-day service, 365 days a year, between 8am and 8pm. Patients discharged from hospital into Home First receive step-down intermediate care with community-based assessments and interventions at home to support recovery and monitoring in the early days post discharge to prevent re-admission.

Responsibilities
* Provide personal, health and therapy care for people across Sussex with acute and diverse needs.
* Assist multi-disciplinary colleagues in ongoing assessment and provision of care and reablement as prescribed within the personalised care plan.
* Perform simple clinical and therapeutic procedures such as taking physical observations, venipuncture, administering medications, simple wound care, rehabilitation programmes and equipment provision.
* Observe changes in patients’ status and communicate updates to the team with accurate record keeping.
* Support training and development through on‑the‑job learning; the role includes basic nursing and therapy skills with progression toward Care Certificate and competency development.
Qualifications and Training
* No previous experience required; on‑the‑job training is provided.
* Training will cover basic nursing and therapy skills, with support to achieve the Care Certificate.
* Structured competency framework to support continuous development; potential career progression to Nursing Associate or other degree‑level healthcare roles with appropriate qualifications.
