About the Role
Our team is seeking a highly motivated practitioner passionate about improving the experience of our ageing and frail population. The successful candidate will have knowledge and experience managing patients with high clinical frailty scores and complex care presentations. Excellent communication skills are essential, especially in discussing clinical conditions and anticipatory care planning. The role involves working across organizational boundaries in partnership with other health and social care teams.
Responsibilities
1. Utilize Comprehensive Geriatric Assessment for screening frailty and planning multidisciplinary care.
2. Prioritize care and conduct daily medical reviews of frail patients alongside the multidisciplinary team.
3. Demonstrate good organizational skills, excellent communication, and a commitment to professional development.
4. Work variable shifts depending on the location.
Training and Development
We may consider applications for trainee posts, supporting development to meet advanced practice competencies through academic courses, supervision, and clinical practice. Applicants should have relevant coursework for the advanced practice pathway.
Qualifications
Essential for Band 7:
* HEI-accredited Independent Prescribing Qualification - V300
* HEI-accredited Clinical Examination & History Taking Course
* HEI-accredited Decision-Making Module
Highly Desirable:
* Previous advanced practice experience
* Experience in autonomous practice within a frailty setting
Additional Skills/Qualifications
* Postgraduate Qualification in Advanced Practice
