About the Role
We are looking for an experienced Nurse to work in a supportive environment where the care of patients comes first. This role is based within our friendly and supportive Community Team and involves visiting people in their own homes to assess, plan care and provide support when they need it most.
Shift Details
* Shift times: 08:00 – 20:00
* Working 1.5 weekends per 4‑week period
Responsibilities
* Provide community‑based clinical care, assessing, planning and delivering care in patients' own homes.
* Work closely with the multidisciplinary team—including specialist doctors, therapy staff, volunteers, nurses and other MDT members—to deliver holistic care.
* Support patients and their families in a holistic approach to achieve best possible outcomes, including end‑of‑life care.
Essential Qualifications
* NMC registered nurse with UK post‑registration experience in the clinical field.
* Experience delivering or managing community‑based clinical care.
* UK driving licence and access to a vehicle.
* Skilled in implementing care and access adjustments.
* Strong multidisciplinary communication skills.
* Ability to travel independently.
* Evidence of continued professional development.
* Knowledge of current legislation in the independent care sector.
* Knowledge of best clinical practice for the client group.
* Demonstrated experience of practising in a self‑directed and highly motivated manner.
* Capacity to build, lead and influence an effective team in a clinical setting.
* Completion of NMC recognised accredited mentor training or willingness to undertake it.
Desirable Qualifications
* Non‑medical prescribing qualification or interest in developing it.
* Experience of working in Palliative and End‑of‑Life Care (e.g. hospice, high‑dependency unit, oncology or community service).
* Comfortable leading junior staff and providing guidance and support.
Induction and Training
Sue Ryder provides a thorough induction with supernumerary time to settle into the role. Full training and competency assessment will be provided.

Other Information
Hours: 37.5 hours per week. Pay: £16.94 – £18.71 per hour, depending on experience, plus enhancements and competitive benefits.
