Assistant Practitioner Therapy – Frailty Team

The Assistant Practitioner Therapy role is within the y therapy Frailty team is to assess patients and provide treatment and discharge plans. The role involves working across both teams to support patients in their hospital Journey and assist in a rapid and safe discharge. The role is for a dual trained assistant practitioner working closely with physiotherapists, Occupational therapists, and assistant practitioners. The aim is to provide short-term assessments, and plan for discharge from hospital.

A Band 3 to 4 development would be considered.

The successful candidate will work 08:00-20:00 across a 7-day service and work within a competency-based framework to manage a caseload of clients. The role involves gaining baseline information from a variety of sources to enable comprehensive client assessment, working independently or jointly with qualified therapists (dependent on level of complexity) to fully assess client needs including physical, functional, social, and cognitive aspects. The candidate must be able to plan, review, and modify treatment directly in line with client goals and outcomes achieved per intervention and take responsibility for managing their own caseload whilst proactively accessing regular supervision and support to do so. The candidate must ensure every contact counts and gain supervision for tasks which are beyond competencies completed.

It is essential to work closely with other members of the MDT to provide holistic person-centred care, actively engage in training, supervision, reflection, and annual reviews, be proactive in accessing support when needed, managing risks as they occur, and being responsible for ensuring an appropriate level of activity for each client. Other duties may include performing audits, developing resources and ways of working in line with current research.
