Job overview

The post holder will be an active member of the multi-disciplinary team, supporting Registered Nurses, Dialysis Assistants, with delegated clinical care duties including clinical observations as directed by registered professionals and within clearly defined parameters, to provide healthcare and support that is individualised to the patient needs. Assisting the RN in the preparation of the area for dialysis.

Applicants must have Experience of working in health or social care and experience of undertaking and recording clinical observations and escalation of changes or findings.

Applicants must have:

1. Numeracy and Literacy Level 2 (GCSE A-C/4-9 equivalent)
2. Care certificate
3. Level 3 vocational qualification:
or Short courses and completion of competency workbook equivalent to Level 3 in a range of patient and clinical care duties and/or Equivalent knowledge and experience in a range of clinical care duties equivalent to Level 3;
- Theoretical knowledge
- Experience of a range of care and related procedures
- clinical observations
- knowledge of relevant legislation

Main duties of the job

The post holder has a personal responsibility and duty of care to patients/service users; they are expected to comply fully with all Trust policies and protocols, including those related to patient safety and the prevention of healthcare associated infections.
They must also promote dignity, privacy, and respect, to deliver compassionate, safe, high-quality patient care.

Essential criteria

4. Excellent interpersonal, communication and active listening skills. Ability to adopt appropriate styles and methods of communication.
5. Time-keeping, planning and prioritisation skills.
6. Basic IT skills to use electronic patient records and other Trust systems.
7. Analytical and judgement skills to make decisions based on patient / service user interactions and observations and metrics.
8. Working knowledge of legislation underpinning care provision
9. Ability to demonstrate the importance of holistic personcentred care and related procedures.
10. Understanding of the importance of robust, accurate, clear, timely documentation.
11. Attention to detail.
12. Insight into behaviours and skills of a carer and the need for effective team work.
13. Knowledge of Safeguarding
14. Understanding work-related boundaries.
15. Awareness of GDPR and the need Familiarity with current NHS and social care policies. Working knowledge of STHFT policies and procedures. All will be assessed through application and Interview 7 for confidentiality

16. Numeracy and Literacy Level 2 or Maths & English GCSE (English Language or English Literature) A-C/4-9 or Maths & English O Level/GCE A-C or Maths & English CSE Grade 1 or NARIC/ENIC which compares the qualification to UK GCSE (
17. Care Certificate (if you dont hold this certificate, you would be expected to complete this during your first 6 months of employment)
18. Disclosure and Barring Checks (DBS)
It is the policy of South Tees Hospitals NHS Foundation Trust that all successful candidates who are new to the Trust who require DBS clearance for the post they have been offered, are required to pay the cost of their DBS.
