What you will do: Build meaningful connections as you identify and engage with rough sleepers, offering essential support to service users across Brighton. Act as the main care coordinator for specific service users, conducting screenings, asset mapping, and recovery planning, using motivational interviewing techniques. Coordinate and implement an individualised care package including psycho-social interventions, one-to-one key work, and group sessions. Complete comprehensive service user assessments, including mapping community resources, and develop recovery plans. Support and develop service-based recovery communities, fostering positive relationships within the community. Perform risk assessments and manage risk, ensuring the safety and well-being of service users.
