Designated ACP within the Older Peoples Pathway. The post holder will work 0.5 wte for the Bedfordshire & Luton Dementia Intensive Support Service, focusing within Luton. The ACP will practice as a Senior Clinician (80% time commitment) with the service, while having responsibility for aspects of Leadership, Research and Education ( 20% time commitment) as defined by the Service Job Plan.
Responsible for the clinical assessment, investigation, diagnosis and treatment and/or referral of patients presenting with Dementia who require the intensive support of the DISS Service, assessing with a view of making a diagnosis (if appropriate) within care services, identifying complex and specialist needs and develop, implement and evaluate plans of care.
Working as a senior member of the Clinical Team, the Advanced Clinical Practitioner has a high degree of autonomy, the post holder will manage undifferentiated and undiagnosed Mental Health conditions using advanced clinical skills within own sphere of competence.
The post holder will work collaboratively with the team and external providers, to ensure the right care, by the right person, at the right time and in the right place. Practitioners will aim to enable individuals to maintain or regain independence or ensure a plan is in place for them to achieve their optimum personal potential safely and to prevent unnecessary or extended hospital admission.
Main duties of the job
• Work towards health promotion and prevention and comprehensively assess and manage patients for risk factors and vulnerabilities.
• Draw on a diverse range of knowledge in their decision-making to establish correct diagnosis and determine evidence-based therapeutic interventions, which will include psychological therapies and prescribing medication where legislation permits and within their sphere of competence allows and actively monitor the effectiveness of therapeutic interventions
• Plan and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimize health outcomes and resource use, and provide direct support to staff, patients and clients
• Work in partnership with patients and carers promoting a recovery focused model, home treatment as an alternative to hospital admission
• Participate as a Senior Clinician at daily DISS meetings.
• Provide Senior Nursing Clinical and professional supervision and action learning set across the Older Peoples Mental Health & Dementia Pathway
• Offer training and education on the care, treatment and needs of people with Mental Illness, Dementia and Cognitive Impairment, to junior colleagues other professional & partners. Including bespoke training to Student Nurse training in collaboration with Bedfordshire University
• Lead on coordinating student nurse placements within the Older Peoples Pathway
