Role Overview:
The role of the Case Manager is central to a clients’ journey throughout their treatment at Orri. The role ensures clients have a consistent experience by coordinating all aspects of the clients care across the MDT and provides a single point of contact for the client, the people who support the client and for professionals both within the service and externally. They work with the client to support them from admission into induction and stay alongside them throughout their time in Orri up to discharge.
Main Duties:
1. To introduce themselves to the new client and their carer/friend/partner (where appropriate) on admission into Orri.
2. To ensure new clients receive their full induction by the clinical team.
