Job Summary
We are looking for a motivated and enthusiastic Clinical Pharmacist. We are offering an exciting opportunity for a Clinical Pharmacist to be part of a growing, dynamic, forward thinking, friendly and supportive team. The role is four days a week (30 hours with a 30 minute unpaid lunch). We welcome applications from both experienced Pharmacists and recently qualified Clinical Pharmacists who have completed the CPPE Primary Care Pharmacy Education Pathway.
Main duties
* Support and work alongside prescription admin and pharmacists to respond to medication-related queries, repeat prescriptions requests, medicines reconciliations of discharge and clinic letters and clinical medication safety audits.
* Undertake Structured Medication Reviews via face-to-face or telephone consultations on identified stratified-risk patients including residents in care homes, those with long-term conditions, and/or learning disabilities.
* Adhere to South East London Medicines Management Local and National guidelines, in addition to GP Surgery Policies (confidentiality, safeguarding, lone working and information governance) to support and promote medicines optimisation.
* Proficient in EMIS Web, Ardens Manager, Word Office, Word Excel, Docman 10 and AccuRx.
* Duties will vary according to the demands on the practice as agreed with the GP Partners.
About Us
Lyndhurst Medical Centre is a 4 partner PMS practice with a patient population of just over 12,000. In April 2026, Lyndhurst Medical Centre will be amalgamated with Bursted Wood Surgery, another local APMS practice that are also based in Barnehurst. The total patient population will bring us to around 19,000 and the new practice will work across the current two sites. Both practices come in the top ten in the 2025 National patient survey for overall satisfaction within the Bexley Borough.
We have a fantastic multi-disciplinary team including Salaried GPs, Practitioners, Practice Nurses, Pharmacists, Home Visiting Paramedic, HCAs and a good established support and reception team in place. We are a member of an innovative high performing Primary Care Network (APL Bexley PCN) with four other local practices. We are a friendly practice who highly value both our patients and staff. We believe in developing our staff and offer CDP opportunities.

Job Description
Job responsibilities

* Promote and prescribe high quality, cost-effective and evidence-based medicines.
* Identify and produce recommendations for medication optimisation for senior clinical pharmacists, GPs and other multi-disciplinary team members.
* Work with care home staff to improve the safety of ordering medicines and administration if required.
* Work to encourage good management of common/minor/self-limiting ailments, including promotion of self-care and homely remedies.
* Contribute to the safety of drug monitoring processes and drug alerts
* Analyse, interpret and present medicines data to highlight issues and risks to support decision making
* Advocate and implement local, regional and national formulary recommendations, including public health campaigns.
* Liaise with all members of the local PCN including community pharmacies and community nursing team, secondary care providers and regional CCG teams on prescribing-related matters.
* Aim for good collaborative working practices and developing relationships with other professionals across PCNs and the wider health and social care system.
* Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities
* Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health.

Person Specification
Qualifications
Essential
* Master of Pharmacy (MPharm) degree, followed by a one-year foundation training year
* Hold a registration with the General Pharmaceutical Council (GPhC).
Desirable
* Completed the CPPE Primary Care Pharmacy Education Pathway.
Experience
Essential
* Have good interpersonal skills with the ability to collaborate with multi-disciplinary teams
* High level communication skills and team working skills
Desirable
* Experience of working in a GP Surgery preferable but not essential
