Overview

We are looking for an enthusiastic Community Nurse to join our Richmond Early Intervention Service (EIS). Applicants need to be professionally qualified and committed to providing responsive and high-quality clinical services to mental health service users and their carers.

The Early Intervention Team provides a service for those people with first onset psychoses between the ages of 17 and 65 years. The aim of the service is to provide intensive care using the Assertive Community Team model, to prevent relapse, reduce the number of "revolving door" admissions, ensure effective treatment for resistant symptoms, reduce the incidence of suicide, and maximise an individual\'s personal and social functioning.

The post holder will work within a multi-disciplinary team comprising Doctors, Psychologists, Social Workers, CPNs, OT, Support and Recovery Workers, Employment Specialists, a Team Manager and team administration staff.


Responsibilities

* This role is for a Care Co-ordinator/Key Worker to manage a caseload of clients aged between 18 and 75 who experience serious and enduring mental health illness/s, using evidence based /client centred principles to assess, plan, implement and evaluate interventions.
* Applicants must have a keen interest in all mental health issues and therefore is required to have up to date knowledge of treatments and protocols. The post holder will be able to work autonomously, exercise clear and coherent decision-making skills and be able to work flexibly.
* The role involves working closely alongside the patients and their carers as well as various relevant agencies, including GP\'s, acute Hospitals, Social Services, as well as voluntary support services.
* Working autonomously within the parameters of the role leading and supporting junior staff and wider care team to implement the delivery of quality nursing care, contributing to the physical, psychological, social, recreational and spiritual care in a recovery-focused way through therapeutic engagement and activities, and taking steps to address any issues that arise.
* Leading on and undertaking activities to ensure that the fundamentals of care are met - nutrition, hydration, hygiene, comfort, emotional and social support.
* Co-ordinating and evaluating collaborative patient centred care; getting feedback and improving plans with the multi-disciplinary team (MDT).
* Providing accurate information about care in an accessible format to patients and their relatives / carers.
* Liaising and working positively with members of the multi-disciplinary team and partner organisations.
* Administering, and monitoring the side-effects of, medication; monitoring service user compliance.
* Safety; safe custody of medicines, sharps, clinical equipment, report all incidents following trust policy.
* Leading and supporting/teaching junior staff on risk assessment and risk management that involves making decisions based on knowledge of the research evidence, knowledge of the individual service user and their social context, knowledge of the service user\'s own experience and clinical judgment.
