Job Summary
Health Facilitation and Liaison role providing support to people with a learning disability to access primary care. This period of Service Transformation requires a flexible, responsive service that reflects the changing needs of this population.
Key Responsibilities
* Provide a health facilitation service for individuals with a learning disability who require access to primary care.
* Collaborate with secondary care providers to ensure reasonable adjustments are made in the delivery of care.
* Deliver training to primary‑care staff on the needs of people with learning disabilities.
* Directly assess patients referred to the service from acute and secondary care and provide advice on making reasonable adjustments.
* Work closely with Health Check Champions.
Main Duties
* Ensure people with learning disability receive an equitable healthcare experience.
* Provide direct support when accessing health services.
* Work to reduce health inequalities and avoidable deaths for this group.
* Liaise with a wide range of professionals to provide high‑standard care.
* Deliver training to acute and primary care staff on the needs of people with learning disabilities.
* Collaborate with Health Check Champions to maintain service quality.
Qualifications & Knowledge
* First level nurse: RNLD or RGN.
* Evidence of continuing professional development at postgraduate diploma level or equivalent experience.
Desirable Qualifications
* Degree at first level.
* Teaching qualification.
* Nurse prescribing.
Essential Experience
* Significant experience working with people who have a learning disability.
* Post‑registration experience.
* Experience working as part of a team.
* Experience training others.
Desirable Experience
* Experience working in partnership with others.
* Experience being involved in research or projects.
Language Skills
* Welsh speaker (Level 1) – desirable. The ability to speak Welsh is desirable; English speakers are also welcome.
Employment Details
* Band: 6.
* Salary: £39,263 to £47,280 per annum (pro rata if part‑time).
* Contract: Permanent.
* Working pattern: Full‑time and Part‑time.
* Hours: Various full and part‑time hours are available.
