What You’ll Do:
* Provide skilled in-home nursing visits based on the plan of care
* Perform vital signs, medication administration, wound care, and catheter care
* Observe and report changes in patient condition to the RN Case Manager
* Educate patients and caregivers on medications, safety, and disease management
* Complete accurate and timely documentation
* Communicate effectively with the care team
What We’re Looking For:
* Active Texas LVN license (required)
* Home health experience preferred
* Strong clinical and communication skills
* Ability to follow care plans and adapt to patient needs
* Reliable transportation & valid driver’s license
* Kinnser/WellSky EMR experience (preferred)
