What you will do:
* Undertake strengths-based assessments of adults’ needs, risks and outcomes.
* Deliver early intervention and preventative support to avoid crisis or long-term care.
* Develop and review care and support plans promoting independence.
* Work in accordance with the Care Act 2014 and relevant legislation.
* Support individuals to access local community services and resources.
Person specification:
Qualifications:
* Degree or equivalent qualification in Social Work
Registration & Compliance:
* Registered with Social Work England
* Enhanced DBS
* Right to Work in the UK
Key Skills:
* Strong assessment and early intervention skills
* Good understanding of Adult Social Care legislation, including the Care Act
* Ability to work in a strengths-based and person-centred way
* Excellent communication and partnership working skills
* Strong organisational and case management abilities
* Ability to work independently and within a team
Experience:
* Post-qualification experience in Adult Social Care
* Experience within early intervention, prevention or locality teams
* Experience supporting adults with a range of needs
* Experience of multi-agency partnership working
