The Home First service brings together nurses, therapists, health and therapy assistants to work with the acute Multi-Disciplinary Team & Transfer of Care Hubs to promote an efficient, effective discharge from hospital to the patient's own home.
Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Patients discharged into Home First receive step-down intermediate care, involving community-based assessments and interventions in their own homes to support recovery and monitor health post-discharge to prevent re-admission. Home First also provides acute reablement for up to 10 days, supporting patients to achieve functional goals and work towards independence in activities of daily living.

Main duties of the job
As a Senior Nurse, you will conduct holistic assessments, plan, implement, and evaluate treatment care plans, and review care provision.
You will be responsible for staff management, supervision, development, shift coordination, visit allocation, and performance reviews.
You will utilize your clinical expertise to support patients and the team, including completing assessments, planning and delivering care such as medication management, wound, and catheter care, and identifying deteriorating patients.
As part of the team, you will receive support from multidisciplinary colleagues and participate in in-service learning. You will also supervise and support the development of colleagues and pre-registration students, developing your specialist areas of practice.
You will contribute to service development through reflective practice, clinical supervision, and quality improvement initiatives. This role offers opportunities to develop leadership skills through formal training, shaping future clinical practice.
