Overview
We have an exciting opportunity for a Band 6 Occupational Therapist post within the Rural Alliance Care Community Team based in Chowley Oak as part of an integrated care team.
Eligibility
The role is suitable for an experienced Band 6 Occupational Therapist or a Band 5 occupational therapist seeking progression into a community setting.
Working Hours
Part‑time 30 hours per week.
Responsibilities
* Assess and treat patients referred with complex rehabilitation needs, working autonomously as a specialist therapy practitioner within an integrated Care Community Team.
* Provide therapy to patients in their own homes, supporting rehabilitation and decisions around remaining in community.
* Operate as a member of a multi‑disciplinary team, communicating patient information to health and social care agencies and planning effective care.
* Participate in departmental in‑service training, journal club activities and the CPD programme to consolidate knowledge and specialist clinical skills.
* Provide supervision, teaching and support to senior and junior staff, assistant staff and act as a clinical educator for undergraduate students.
