Personal Attributes
Essential criteria
1. Ability to work under pressure, individually and within a team
2. Able to prioritise own caseload
Knowledge
Essential criteria
3. Knowledge complex discharge planning process.
4. Understanding of assessing and problem solving issues related to discharge planning
5. Understanding of multidisciplinary assessment and treatment for older people / vulnerable adults within acute care and community settings.
Qualifications
Essential criteria
6. Good standard of general education
7. NVQ level 3: health / social care management or equivalent
Technical Skills Competencies
Desirable criteria
8. Use of IT clinical systems ie: e-CaMis Ena, discharge databases
Experience
Essential criteria
9. Experience of working in a hospital setting
10. Understanding of complex discharge planning in care of the elderly, stroke, long term conditions and vulnerable adults.
Desirable criteria
11. Previous experience of working with patients requiring CHC and Social Services involvement.
