Derbyshire Adult Care Prevention and Personalisation team are looking to recruit a qualified Social Worker based in Erewash First Response Team, Ilkeston.
The team is based in the community consisting of a mix of Social Workers and Community Care Workers and Occupational Therapists who work closely with reablement care staff and other health colleagues in a multi-disciplinary model.
The aim of our team is to carry out initial and full Care Act assessments, planning, intervention, reviews, safeguarding’s and mental capacity assessments to support reablement and identify eligible needs for long term support.
This is a fast pace team and there is an expectation that the successful candidate will be able to manage a caseload and support on duty as required.
