Palliative Care Clinical Nurse Specialist

We are seeking a compassionate and skilled Specialist Palliative Care Nurse to join our team in delivering high-quality, patient-centered care. This advisory role involves supporting patients with cancer and non-malignant conditions, as well as their families, by providing expert guidance on symptom management, psychosocial support, and end-of-life care.
As a key member of the multidisciplinary team, you will act as a resource for clinical colleagues across the Trust, offering specialist advice on palliative care issues. You will also play a vital role in education and training, facilitating learning for staff at all levels and contributing to the development of evidence-based palliative care programmes.
This is a 7-day service, and the successful candidate will participate in a face-to-face visiting rota operating from 9am to 5pm.
Main duties of the job
Clinical Excellence
* Deliver evidence-based, individualized care through holistic needs assessments, planning, and evaluation.
* Act as a Keyworker for specialist palliative care patients.
* Facilitate sensitive discussions and ethical decision-making around end-of-life issues.
* Provide clinical expertise, specialist advice, and support across service boundaries, including medical, allied health professionals, and other multidisciplinary team members.
* Liaise with Cancer Services, Community Palliative Care teams, local Hospices, GPs, and other community health professionals.
* Collaborate across professional boundaries, using creative reasoning and problem-solving to improve patient care pathways.
* Manage change within complex environments, evaluating its impact on patient care.
* Promote patient experience feedback through local initiatives to foster innovation and improvement.
Leadership
* Demonstrate high standards of care as a role model and provide clinical leadership.
* Offer specialist education and training to healthcare professionals involved in patient care.
* Lead the development, management, and promotion of the specialist nursing service to ensure high-quality, cost-effective care.

Person Specification
Education and qualifications
* Relevant professional qualification and registration/Registered general nurse
* BSc/MSc degree in palliative care or similar, or willingness to undertake
* Evidence of ongoing professional development
* Advanced communication skills training or working towards it
* Teaching qualification
* Experience in specialist palliative care and ability to work at Band 7 level
* Experience of teaching
* Leadership and management experience
* Ability to conduct reflective practice and participate in clinical supervision
* Experience of multi-professional working
* Evidence of advanced communication skills
* Ability to lead and influence change
* Previous experience as a clinical nurse specialist
