Right now, we’re looking for aPhysiotherapist with community experience to join the Community Hub, RapidResponse team. This service provides a multidisciplinaryapproach to patient care and is focused on, hospital avoidance, earlysupportive discharge (from hospital) and non-acute rehabilitation in a homecaresetting
The Physiotherapist(s)in this team work collaboratively with the Emergency Department, GeneralPractice, Residential Aged Care and are part of a team that includes,Registered Nurses, Occupational Therapists and the Health Care Assistants workunder the delegation of the Healthcare professionals.
This role will suitsomeone who is able to work with a wide group of healthcare professionals, havethe ability to practice independently and use prior expertise to ensure patientand client centered care enabling them to maximize independence.
To be eligible for this role, NewZealand Registration and a current Annual Practising Certificate is requiredalong with a current full driver license.
Other requirements for success inthis role includes:
* Previous community experience and/or apassion for providing care that supports people in their own homes
* Ability to prioritise workload, superviseand delegate to care staff
* Delivery of a safe and effective patient focused service
* Have the ability to work with patients and their whanau andfamily
* Have a personal commitment to professional nursing developmentand continuous quality improvement.
