Community Nursery Nurse

A rare opportunity has arisen for a qualified nursery nurse to work in the High Peaks & Dales locality as part of the 0-19 Children's Community Services. You will be involved in maximising the health of children, families, and the community through health promotion, disease prevention, and other activities designed to improve wellbeing and achieve health gain.

You will work closely with the Specialist Community Public Health Practitioners (SCPHN) to identify and support children to be 'ready for school' and to provide other enhanced, time-specific pieces of work under the supervision of the SCPHN.

The Community Nursery Nurse Team works closely with the Health Visitors and the Locality Manager to provide services in the area from Matlock to Glossop. The base for this post will be either Whitworth Hospital (Darley Dale) or Newholme Hospital (Bakewell) but the successful applicant may be required to work across the locality to ensure service delivery, and also travel across Derbyshire on occasion for training or meetings.

The team are hardworking and work together to coordinate service delivery, communicating well with the Heath Visiting Team to ensure children's needs are met and families receive the support required.


Main duties of the job

Support children and families in their homes and other environments by carrying out developmental assessments and planned individual programmes of care under the leadership of SCPHN. Visit families in a variety of community settings and undertake health promotion activities under the supervision of the SCPHN. Support parents and children with a range of school readiness activities.

Promote health enhancing activities in partnership with other health professionals, statutory, voluntary, community organisations and members of the local population.

Maintain the highest standard of confidentiality and integrity when communicating with service users and the wider public. Communicate with clients, health care professionals and the wider team including local Derbyshire County Council using effective communication skills.

Job responsibilities

To use own initiative to support children and families in their homes and other environments by carrying out developmental assessments and planned individual programmes of care under the leadership of SCPHN

To maintain accurate records as per Trust policies and procedures utilising IT resources

To effectively communicate with clients, health care professionals and the wider team including local Derbyshire County Council

Plan individual programmes of care and interventions in conjunction with the SCPHN and in partnership with the client.

Provide a face to face service to families that is personalised in response across a variety of community settings and the patients home.

Undertake health promotion activities under the supervision of the SCPHN

Provision of enhanced support to children and families

Support children with school readiness activities

Promote health enhancing activities in partnership with other health professionals, statutory, voluntary, community organisations and members of the local population.


Person Specification


Qualifications

* CACHE level 3, BTEC National Diploma in childcare, learning and development, NVQ level 3 in Childrens care, learning 7 development, SVQ 3 Or HNC in childcare
* Evidence of continual professional development relevant to the role
* Understanding of factors that influence health and wellbeing.
* Experience of working in a health care setting

* To be able to work in a variety of settings.
* Ability to work independently
* Good time management skills, IT skills and awareness.
* Calm, mature, and friendly approach.
* Flexible approach to delivering services.
