Working as part of a skilled team of Pharmacists, Pharmacy Technicians and Pharmacy Support Workers, you will provide a comprehensive medicines management service to the community hospital wards, focusing on medicines reconciliation, patient counselling, supporting self administration, and discharge facilitation.
You will be fully supported with your training and development and benefit from the wealth of training available to the Pharmacy team, including clinical and management, and leadership opportunities.

The post does not include participation in weekend, Bank Holiday and late duty rotas.
This is a 12 month Fixed Term / Secondment post for Maternity Cover.
Responsibilities
* Undertake a Medicines Reconciliation for all patients on admission and know how best to obtain accurate and up-to-date information about a patient’s medication using the sources of information available.
* Identify, investigate and resolve prescription discrepancies.
* Interpret and apply information retrieved from medical and nursing notes with regards to medicines reconciliation and prescribing issues.
* Check patient’s own medication (PODs) to aid in problem solving and to adequately manage medicines supply.
* Update EPMA system with appropriate extra information as required to aid prescribing and administration.
* Identify patient’s pharmaceutical care needs, e.g. support with compliance.
* Assist in discharge planning as part of the multidisciplinary team.
* Provide verbal and written medicines related advice and information for patients and/or carers regarding prescribed medication and their devices with the primary aim of improving patient outcomes through medicines optimisation.

The role is fixed-term for 12 months with maternity cover and does not require weekend, bank holiday or late duty rosters.
