West Farm Surgery is looking for an experienced part‑time practice nurse to join our friendly team. You will work alongside a passionate nursing team with strong management and a supportive partnership that is always looking for innovative ways of working.
Salary: Dependent on experience. 24 hours per week – days are negotiable. Mixture of face‑to‑face and telephone appointments. Protected admin time.
Main duties of the job
You will be responsible for the delivery of practice nursing services, working as part of the practice’s multidisciplinary team to provide care within your scope of practice to the entitled patient population. Key clinical areas include health promotion, chronic disease management, health prevention, well‑woman and well‑man clinics, and supporting the management team in reviewing clinical policy and procedure.
Job responsibilities
1. Develop, implement and embed health promotion and well‑being programmes.
2. Implement and evaluate individual treatment plans for chronic disease patients.
3. Identify, manage and support patients at risk of developing long‑term conditions.
4. Provide routine nursing care to patients in accordance with clinical evidence, NICE and the NHS Standards.
5. Request and process pathology services.
6. Deliver chronic disease clinics and refer patients to secondary/specialist care.
7. Maintain accurate clinical records in line with legislation.
8. Ensure read codes are used effectively.
9. Maintain chronic disease registers.
10. Develop and embed well‑woman and well‑man clinics.
11. Chaperone patients where necessary.
12. Assist GPs with minor surgery when required.
13. Prioritise health issues and intervene appropriately.
14. Support the team in dealing with clinical emergencies.
15. Recognise, assess and refer patients presenting with mental health needs.
16. Implement vaccination programmes for adults and children.
17. Support patients in the use of prescribed medicines or over‑the‑counter medicines within your scope of practice.
18. Liaise with external services/agencies to ensure patients receive appropriate support.
19. Delegate clinical responsibilities appropriately.
20. Support the clinical team with safeguarding matters.
21. Understand practice policies for substance abuse and addictive behaviour, referring patients appropriately.
22. Deliver opportunistic health promotion where appropriate.
Secondary responsibilities
* Act as audit lead, utilising the audit cycle.
* Monitor and ensure safe storage, rotation and disposal of medicaments.
* Support junior and senior members of the nursing team.
* Participate in local initiatives to enhance service delivery and patient care.
* Support and participate in shared learning.
* Review clinical practices and respond to national policies and initiatives.
* Participate in root‑cause analyses of significant and near‑miss events.

* Postgraduate diploma or degree in Primary Care
Experience
* Experience in a primary care environment.
* Experience in chronic disease management.
* Interest in Diabetes.
