Responsibilities Assist social workers in conducting client assessments and interviews to gather relevant information. Support the development and implementation of individualized care and support plans. Maintain accurate and up-to-date case records and documentation in compliance with organizational policies. Coordinate and liaise with community resources, service providers, and agencies to facilitate client access to services. Provide direct assistance to clients by offering emotional support and guidance as needed. Participate in multidisciplinary team meetings and contribute to case discussions and planning. Monitor client progress and report any concerns or changes in circumstances to supervising social workers. Assist with organizing outreach programs, workshops, and awareness campaigns related to social services. Ensure confidentiality and adhere to ethical standards in all client interactions. Requirements A diploma or degree in Social Work, Psychology, Human Services, or a related field. Previous experience or internship in a social services or community support role is preferred. Strong communication and interpersonal skills to engage effectively with clients from diverse backgrounds. Demonstrated ability to handle sensitive information with confidentiality and professionalism. Proficiency in maintaining case records and documentation with attention to detail. Ability to work collaboratively as part of a multidisciplinary team and independently when required. Good organizational and time management skills to prioritize and manage multiple tasks.
