Job Summary The Disability Support Worker plays a crucial role in providing assistance and support to individuals with disabilities, helping them lead fulfilling lives. Duties - Assist clients with daily living activities, including personal care, meal preparation, and household tasks. - Develop and implement care plans tailored to each client's specific needs and preferences. - Provide emotional support and companionship, fostering a positive environment for clients. - Communicate effectively with clients, families, and other professionals involved in the care process. - Drive clients to appointments or community activities as required. - Maintain accurate records of client progress and any changes in their condition. - Utilize IT systems for documentation and communication purposes. - Participate in training and development opportunities to enhance skills and knowledge in disability support.
