We are looking for a motivated and enthusiastic mental health professional to join the Chester Community Mental Health Team. This role is suitable for a Qualified mental health Nurse and involves working within a multi-disciplinary team to deliver safe, effective and meaningful evidence-based interventions for our patients. The team values development, collaboration, and a whole-team approach to care, with a clear focus on safety for patients, staff and colleagues.
The role includes supporting clinic duties alongside key-working a varied caseload, within a service that has recently undergone positive changes and remodeling to improve delivery of services.

Responsibilities
* Undertake assessments and coordinate the care of patients with functional and organic mental illness within a multi-disciplinary framework.
* Support and/or lead aspects of busy community clinics and key-work a caseload, including ongoing assessment, formulation, care planning, risk assessments, reviews and delivering tailored interventions to improve wellbeing/outcomes for individuals and their families.
* Signpost to other agencies where necessary and maintain effective communication and documentation (report writing and assessment skills are essential).
* Contribute to a safe, patient-centered service with robust safety systems and a commitment to evidence-based practice.
Qualifications and Experience
* Essential: Appropriate Professional Qualification; evidence of continuing professional development; CMHT experience; minimum of 2 years post-qualification experience in a community mental health setting at Band 5 or 6.
* Desirable: Mentor qualification; computer skills; experience of implementing significant changes in working practices; experience of monitoring standards of care; experience of providing supervision; experience of working within a multidisciplinary team; working knowledge of memory and functional disorders.
Knowledge and Skills
* Specialist knowledge relevant to the service; working knowledge of relevant legislation; evidence of undertaking audit work.
