Job summary

An exciting opportunity has arisen for a Specialist Nurse in our welcoming and experienced Vascular team. The role would include the provision of pre and post operative management of Vascular surgery patients as part of the multi-disciplinary team. The role provides opportunity to work autonomously, carrying out comprehensive vascular assessments and developing plans of care for both inpatient and outpatients.

Any new team member will be required to work closely with community teams and with our partners at Addenbrookes Hospital.

Main duties of the job

Assessment, planning, implementing, and evaluating care, as an autonomous practitioner with specialist skills.

This includes:

o Providing specialist psychological, social, and practical support that is responsive to the perceptions and needs of patients and their families.

o Act as the first point of contact for patients during investigation, diagnosis and treatment for those patients referred to, and who are currently under, the care of the Vascular service.

Provision of care through accepting referrals, undertaking therapeutic procedures, physical assessments and obtaining written consent to treatment

Referring patients directly to specialists in other professions

Providing expert specialist clinical advice for patients, carers and health care professionals

Carrying a caseload, and responsibility for the management of complex cases, including nurse led clinics

Improving and maintaining standards of care within the specialty

Liaising with all appropriate members of multi-disciplinary team and ensuring a seamless service between acute and primary care services

Lead in patient education and counselling. This will include maintaining up to date literature and resources for both patients and staff.

Where applicable and qualified, act as a Nurse Independent Prescriber, prescribe any drugs within clinical competencies
