Job responsibilities Demonstrate and maintain consistently high standards of individualised palliative nursing care and support under the guidance of the Community Nursing Specialist. Work alongside the patients family and / or professional carer, maintaining an approach that incorporates physical, psychological, social and spiritual aspects of care, in accordance with the Hospice of St Francis philosophy. Observe and report changes in the patients symptoms, condition and circumstances to the Hospice Community Nursing Team, notifying other healthcare professionals as necessary. Assess patients physical and emotional needs, utilising assessment tools such as IPOS and under the guidance of the Community Nursing Specialists offer basic symptom control advice and support and review outcomes. Independently manage, with remote support from community nursing team, patient and carer emergencies within the community setting. Maintaining a calm and professional manor whilst acting in a timely and effective way. To take responsibility for the ongoing review of a small group of patients, reporting to community nurse specialist when appropriate for advice and guidance, escalating concerns appropriately within the Hospice and wider community MDT. Maintain concise accurate and legible records of patient/family care in all health care settings, using electronic patient data base, meeting legislative requirements within The Hospice of ST Franciss practices and procedures. Responsible for identifying and acting on any risk of safeguarding to patients and their families in accordance with the Hospice of St Francis Safeguarding of Vulnerable Adults and the Safeguarding of Children Policies and Procedures. Offer bereavement support, either face to face or over the phone, referring as appropriate to the bereavement team and highlighting any bereavement concerns to The Community Nursing Specialist Assist with the patients nutritional requirements including the preparation of light meals and drinks according to dietary requirements and choice. When required assist with patients personal hygiene and toileting, which may include the use of bedpans, commode, lavatory, catheter or conveen. Administer medication in accordance with the Hospice of St Francis policy and procedure where relevant training has been undertaken. Undertake simple dressings as prescribed in the patients care plan whilst maintaining best practice in management and control of infection. To support patients with rehabilitation in their home setting, working on goal setting and motivation. Ensure a safe environment to work within, and report in a timely manner to the Community Nursing Services lead if concerned.
