The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated and integrated approach to planning, development, and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Queensland Region.

The Australian Family Partnership Program (AFPP) is a home visiting program for first-time mothers of Indigenous babies. AFPP Nurse Home Visitors, together with Indigenous Family Partnership Workers, support women from pregnancy through to children’s second birthdays. During the program, women have an opportunity to build parenting capacity, explore strategies for achieving identified goals, and feel empowered to manage their lives. The underlying objective of the AFPP is to improve the health, wellbeing, and self-sufficiency of parents and their children.

About the Role

The AFPP Nurse Home Visitor (NHV) works in partnership with the Family Partnership Worker to provide a culturally safe, comprehensive home visiting service to women pregnant with an Aboriginal and/or Torres Strait Islander baby and their families. The Nurse Home Visitor is responsible for maintaining the highest standards in nursing practice, meeting AFPP model requirements, and observing the Primary Health Care Service’s policies and guidelines. The Nurse Home Visitor reports to the AFPP Operations Manager.

1. Works collaboratively with teams and uses Reflective Practice in supervision to improve skills in delivering care in line with the Program model.
2. Utilizes the nursing process and delivers content at home visits in a way that maintains alignment with client-centered principles, including modeling therapeutic relationships, change theory, self-efficacy, client-centered, strength-based, and solution-focused approaches.
3. Carries a caseload of up to 20 clients.
4. Maintains accurate client service data, timely input of data into the Data Collection system, and participates in team-driven regular review and analysis of data reports to ensure Continuing Quality Improvement (CQI).
5. Provides education, support, and referral resources when assisting women and their families to identify and achieve their stated goals and evaluate AFPP outcomes.

About You

* AHPRA Registration
* Current Criminal History Check or willingness to obtain
* Qualification in Nursing
* Current C Class Drivers Licence (Qld)

Desirable, but not mandatory

* Demonstrated experience working with Aboriginal and Torres Strait Islander people and/or communities
