We are currently recruiting a Specialist Pharmacist to join our forward‑thinking, friendly Health & Justice team at HMP High Down.
About the role
The Specialist Pharmacist will be part of the Prisons Health Care Team, looking after the healthcare needs of patients in secure environments and making a real impact on their health and well‑being. Your role will encompass both clinical and organisational responsibilities, providing expert pharmaceutical advice, supporting policy development and ensuring compliance with medicines legislation.
Key responsibilities
* Provide expert advice on pharmaceutical matters relating to primary care, substance misuse and mental health services, including risk management and compliance with medicines legislation and local policies.
* Contribute to the development and review of policy and procedures affecting pharmaceutical use at local and trust‑wide levels.
* Collaborate with dispensing pharmacy staff to support an effective dispensing and supply service.
* Participate in clinical audits, research and evaluation of medicines and pharmacy practice.
* Deputise for the Lead Pharmacist in their absence and lead clinical pharmacy initiatives within the team.
* Ensure all prescriptions and medication administration records are clinically appropriate and meet legal requirements.
* Manage and optimise repeat dispensing services, including medication review clinics and prescriptions for the repeat dispensing system.
* Perform medicines reconciliation for all patients entering reception and monitor for drug interactions and adverse effects.
* Provide clinical pharmacy input to multidisciplinary teams and support patient‑centred medication reviews.
* Assist in the implementation of NICE publications and the Trust Medicines Management Group recommendations.
* Support planning of medication release with the implementation of a dispensing‑for‑discharge system.
* Record and report medication interventions and promote safe medication practices, including the use of Datix for error reporting.
* Adhere to the Trust Formulary and promote cost‑effective prescribing within the prison environment.
* Act as a clinical role model and mentor junior pharmacy staff, demonstrating safe, clinical and cost‑efficient use of medicines.
