What the Job Involves As a Mental Health Social Worker in a community setting, you will provide assessment, care management, and social work services to adults with mental health issues and their carers. You will work within the multidisciplinary Community Mental Health Teams, participating in duty rotas, managing and reviewing care plans, and acting as a care coordinator. You will undertake safeguarding adult inquiries, maintain compliance with local and statutory guidelines, and contribute to the professional development through ongoing training. Additionally, you will monitor packages of care, liaise with various stakeholders, and ensure adherence to the Council's Health and Safety Policy.
