Do you have the drive and passion to make a real difference in the lives of children in care? Are you a proactive, pragmatic, and progressive social worker? We are seeking a Senior Social Work Practitioner to join our dynamic Children in Care Team.
About the Role:
You will manage your own caseload, working with children and young people who are looked after and those with a plan of adoption. You should demonstrate strong analytical skills and formulate outcome-focused care plans. Exceptional organisational skills are essential to manage and prioritise your work effectively and efficiently.
In this role, you can expect to:
* Devise dynamic care plans for children in care within Bath and North East Somerset
* Facilitate adoption processes, ensuring children find permanent, loving homes
* Develop and implement transition plans to support young people towards adulthood
* Support reunification efforts to help children return to their families when safe and appropriate
* Promote family connections and lifelong links, recognising the importance of maintaining relationships with birth families and significant others
