Job Overview

An exciting opportunity has arisen for an enthusiastic individual to join the Digital Care Hub as a Clinical Team Leader. The post holder must be a practising non‑medical prescriber and have an understanding of providing digitally enabled clinical services and their impact.


Main duties of the job

* Provide a high standard of clinical leadership and supervision to the Digital Care Hub service delivery team, ensuring procedures, guidelines and protocols are adopted to guarantee safe, effective care for patients.
* Support staff development to enhance service provision and customer relationships.
* Participate in clinical discussions with stakeholders such as ICB, PCNs, Community Teams, Out-of-Hours Services and care homes.
* Ensure compliance with the NHS Constitution.

Detailed Job Description and Main Responsibilities


Clinical Expertise

* Demonstrate professional accountability and adhere to the NMC or other professional Code of Professional Conduct.
* Take responsibility for the assessment of patients who contact the digital care hub services via telephone and video link.
* Competently assess and triage patients using the digital care hub services, including assessment, planning and evaluation of programmes of care.
* Participate in the multi‑professional review of patients who have been referred to digital care hub services, including those recently discharged from hospital.
* Develop competence of professional practice to advise and support patients using appropriate protocols.
* Develop clinical skills in triage and assessment of patients via the digital care hub, as well as within hospital.
* Supervise and maintain accurate legal records using care planning, Airedale NHS Foundation Trust’s and digital care hub’s documentation.
* Offer help and advice to patients’ relatives and friends responding to their individual needs in a caring and supportive manner.
* Collaborate with ward and community staff to provide the appropriate delivery of patient care and clinical and professional advice.
* Act as a clinical role model, reflecting the values, behaviours and clinical standards of the Trust and digital care hub.
* Adhere to Airedale NHS Foundation Trust’s policies and procedures relating to Health and Safety.
* Consult and co‑operate with clinical and support staff from all areas.


Managerial Skills

* Oversee e‑roster ensuring adequate staffing levels across the team.
* Approve annual leave in line with agreed staffing levels.
* Ensure all PDRs are completed for all service delivery staff within the Digital Care Hub.
* Manage sickness and implement the Trust’s sickness absence policy for the service delivery team.
* Support the DCH Business Manager with recruitment.
* Induct new staff.
* Ensure all staff attend mandatory training.
* Deliver training for new services/existing service developments to all staff.
* Support service delivery staff in professional development.
* Act as a deputy alongside the DCH Business Manager in the absence of the Head of Digital Care Hub and Nurse Consultant.
* Inform the Head of Digital Care Hub and Nurse Consultant of accidents, complaints, untoward incidents and other significant events.
* Ensure the health and safety of hospital personnel, patients, relatives and visitors within the Trust and Digital Care Hub.
* Supervise and manage safe use of equipment and supplies.
* Provide support to all team members, especially in times of stress.
* Support staff in developing confidence and competence in their specialist areas.
* Ensure service delivery staff proactively develop their specialist area, ensuring ongoing service improvement.
* Provide clinical supervision as required.
* Ensure clinical guidelines, protocols and procedures are available, up to date and adhered to.
* Maintain a high standard of clinical care provided by the hub to patients and their carers.


Person Specification


Skills – Essential criteria

* Practising non‑medical prescriber or working towards it.
* Leadership and management skills.
* Excellent written, verbal and electronic communication skills.
* Presentation skills.


Qualifications – Essential criteria

* RN/Project 2000 Adult Branch, RGN or Registered AHP.
* First degree in a relevant subject or substantial experience in Acute or Community Care.
* ENB 998 / C & G 730 / Educational qualification.
* Non‑Medical Prescribing V300 or working towards it.


Qualifications – Desirable criteria

* AIM, ALERT courses desirable (or willing to undertake).
* Masters modules in advanced practice (or willing to undertake).


Knowledge – Essential Criteria

* Extensive clinical knowledge in the care and management of the acutely ill.
* Demonstrates self‑learning to improve knowledge and skill base.
* Knowledge of risk management.
* Knowledge of NHS agenda in relation to Acute Care, Telemedicine, Clinical Governance and the Nursing and AHP Agenda.
* Understanding and ability to implement evidence‑based practice.


Knowledge – Desirable criteria

* Root cause analysis training.
* Knowledge of intermediate care and community services.
* Knowledge of SystmOne and other digital systems used for patient care.


Experience – Essential criteria

* Substantial experience in a senior role within Acute or Community Care.
* Ability to work with and motivate clinical and non‑clinical teams.
* Evidence of sustained practice development in previous employment.


Experience – Desirable criteria

* Previous experience at senior level within a Trust.
* Experience of designing and delivering training and education for health care professionals.
* Experience in managing and delivering sustained change.
