Overview
Band 6 District Nurse to join the Adult Community Nursing Team, working flexibly over a seven day period. The Specialist Practitioner Qualification in District Nursing is desirable or substantial years of experience at band 5 level within the district nursing service.
The Broughton District Nursing Team delivers care between 08:00 and 18:00 in the community of Salford to people in their own homes. We are part of a dynamic, integrated health and social multidisciplinary team delivering high quality care, advice, and support and monitoring to patients, their families and staff within the community settings. You will work under the direction and supervision of the District Nurse Lead to provide care to patients with complex and non-complex needs with the aim of promoting independence and self-management.
You should have excellent communication skills.
You will be supported by an expert team in an environment where training and development opportunities are actively encouraged in order to meet the clinical needs of the role. We offer a comprehensive induction programme and you will be appraised on a yearly basis with a view to developing your knowledge and skills where appropriate.
Responsibilities
* Provide nursing care to patients within the community environment and plan the workload for junior staff and support the team leader.
* Participate in the delivery of nursing care and support to clients within the community setting and work autonomously with confidence and competence.
* Work under the direction of the Team Leader and be responsible for assessment and management of the health needs of an identified population, including end of life care and patients with long-term conditions and/or complex nursing needs.
* Chair team safety huddles, attend multi-professional meetings, discharge planning meetings, and feedback to the team to ensure seamless patient care.
* Provide daily clinical and managerial leadership to nursing and support staff in the delivery of evidence-based practice as a Sister/Charge Nurse.
* Assess patient care needs and develop, implement and evaluate programmes of care for specific patient groups and support care of all patients.
Qualifications and Requirements
* Band 6 District Nurse.
* Desirable: Specialist Practitioner Qualification in District Nursing or substantial experience at band 5 level within district nursing.
* Excellent communication and interpersonal skills.
