Community Staff Nurse Rapid Response

Are you a Registered Nurse looking to make a difference in the community? We are looking to recruit a NMC registered nurse to work within the Rapid Response team in High Peak and Derbyshire Dales.

Main duties of the job
Provide high‑quality nursing care to patients in the local community to keep them safe at home, prevent hospital admission and support hospital discharge home.
Assess, plan, implement and evaluate individualized patient care in the most efficient and clinically effective way, whilst maintaining the patient's privacy and dignity.
Provide advice and support to junior staff in the delivery of high quality care and actively support the Community Nursing sister/charge nurses in the management of the health care environment.
Implement or refer to relevant services as appropriate.
Work in close collaboration with Integrated Community Teams/GP surgeries and local acute trusts.
Respond to priority nursing visits.

Education / Qualifications
* Full NMC registered Nurse
* Evidence of recent relevant training
* Experience of Community Nursing
Experience and Knowledge Required
* Experience of working in the community
* Evidence of research‑based/reflective practice
* Knowledge of local and national priorities
* Understanding of the wider NHS agenda regarding community services
* Experience of long‑term conditions
* Experience of Leg Ulcer management and lower‑limb assessments
* Experience of end‑of‑life care

* Ability to supervise junior nursing staff
* Ability to assess, plan and implement care
* Previous experience of using SystmOne
Other
* Ability to travel across Derbyshire County visiting patients' homes and attending meetings and training
