Thrive Leaving Care is currently seeking a dedicated and compassionate Leaving Care Support Worker to join our team in Bolton. Our service supports up to 6 Young People in a flat scheme model with onsite staff and management.

Position: Leaving Care Support Worker
Location: Darcy Lever, Bolton.
Employment Type: Day, Evening, Weekend and Sleep in shifts. To be discussed at interview.

Responsibilities:
* Provide practical and emotional support to young people leaving the care system, aged 16-18, to help them transition to independent living successfully.
* Assist in developing personalised support plans for each individual, identifying their strengths, needs, and aspirations.
* Offer guidance and encouragement to develop essential life skills, such as budgeting, cooking, cleaning, and maintaining a tenancy.
* Collaborate with other professionals, including social workers, educators, and healthcare providers, to ensure holistic support for young people.
* Facilitate workshops and group activities to enhance personal development, resilience, and social skills.
* Advocate for the rights and needs of young people within the community and relevant agencies.
* Support individuals in accessing education, training, employment, and recreational opportunities.
* Maintain accurate records and produce reports as required.
Qualifications:
* A minimum of 1 year experience working with young people in a care or support setting.
* Knowledge and understanding of the challenges faced by young people leaving the care system.
* Strong interpersonal skills with the ability to build trusting relationships with young individuals.
* Excellent communication skills, both written and verbal.
* Empathy, patience, and the ability to remain calm in challenging situations.
* Ability to work independently as well as part of a team.
* Knowledge of local resources and services available to young people.
* Commitment to promoting equality, diversity, and inclusion.
* Relevant qualifications in social work, youth work, psychology, or a related field are desirable.
