Overview
To participate in the provision of high quality care. This will involve assessing, supporting, and monitoring service users who are experiencing mental health issues. It involves working in a variety of community settings, liaising and co-working with other services (statutory and non-statutory), and engaging with carers and families in their homes, clinics, hostels, supported living, and residential settings.
Responsibilities
* Work as a Care-coordinator or Lead Professional within the team in the planning, delivery, co-ordination, and evaluation of assessments and formulation.
* Work closely and collaboratively with other team members including Therapists, Medical professionals, Nurses, Allied Health Professionals, and support staff.
* Carry out complex person-centred assessments.
* Ensure that all care offered is evidence based and that interventions take into account the physical, cultural, psychological and spiritual needs of service users.
* Assess care needs, devise care programmes and packages of care including risk assessment, Dialog+ and implement and evaluate these in collaboration with individual service users and their carers.
* Support and mentor less experienced staff, support workers, and students on placement within the team.
* Undertake duties designed to extend clinical and managerial expertise as delegated by the Team Manager when required.
* Undertake comprehensive outcome monitoring of all referrals.
