B6 Community Learning Disabilities Nurse
KEY RESPONSIBILITIES:
1. Be responsible for a designated caseload, liaising with other agencies, service users, and carers.
2. Utilize nursing knowledge and skills to assess healthcare needs of people with learning disabilities, identify their healthcare needs, and implement nursing interventions.
3. Evaluate the outcomes of nursing interventions to ensure care plan objectives are met and health gains are achieved.
4. Ensure all interventions are within the scope of clinical practice, maintaining training and skills in line with current guidance.
5. Provide specialist advice and training to paid and unpaid carers about learning disabilities to improve their caring roles.
6. Undertake additional formal training to qualify as a supplementary and/or independent non-medical prescriber, contributing to service development and enhancing client-centered care.
7. Work in partnership with colleagues and other professionals to provide specialist nursing assessments and advice, formulating and implementing comprehensive care plans.
8. Administer and monitor prescribed medication as required, observing effects and side effects as part of overall treatment plans.
9. Offer supervision and guidance to other nurses and professionals as necessary.
10. Support student nurses, TNAs, and other students during placements.
11. Ensure effective communication within the team.
12. Implement risk management strategies to mitigate risks.
13. Contribute to external training to support therapeutic interventions in various settings.
14. Maintain knowledge of the Mental Health Act, CTRs, LAEPs, and the DSR.
15. Facilitate access to other services, including mental health admissions, ensuring appropriate involvement and measurable objectives for admission and discharge.
16. Collaborate with Local Authority and other health providers to facilitate timely discharge to community-based services, following protocols.
17. Advocate for service users and carers, ensuring care needs are met and concerns are escalated appropriately. Promote rights and informed consent, including mental capacity considerations.
18. Facilitate access to independent advocacy services and IMCA where appropriate.
19. Maintain accurate records adhering to regulatory standards, data protection laws, and organizational policies.
20. Ensure detailed care plans, outcome measures, risk assessments, and treatment programs are completed and communicated effectively.
21. Follow and evaluate care pathways and protocols regularly.
22. Be a visible and competent role model, fostering good working relationships with partners.
23. Provide verbal and written reports for meetings such as reviews, safeguarding, and planning.
24. Work with the wider MDT to contribute to community care planning and delivery.
25. Participate in multidisciplinary clinical discussions, supervision, and referral processes.
26. Support flexible working practices as required.
