Overview

Are you a band 6 Physiotherapist/Nurse or band 5 looking for the next step in your career? Do you have community or respiratory experience? If so, we have an exciting opportunity to join our service in North East Essex.

The Community Respiratory Service is an integrated team of physiotherapists, nurses & support staff who deliver pulmonary rehabilitation, COPD optimisation, respiratory virtual ward, home oxygen & respiratory physiotherapy for people out of hospital. That may be in clinics, community venues or people’s homes.

You would be working with a team who are passionate about delivering community care to patients living with chronic respiratory conditions. The team work closely to support each other, problem solve with our complex patients, and work closely with our colleagues in Consultant lead palliative care.

Due to the nature of community work & the community locations for our clinics, a driving licence & access to a car are essential.

This is an exciting time for the service as we review & improve it to meet the standards for pulmonary rehab accreditation & increase uptake on our virtual wards.


Responsibilities

* Focus on pulmonary rehabilitation classes provided in a range of venues across North East Essex; may also carry out home visits to patients for assessment/review within your scope of skills to support effective team working.
* Engage with people, put them at ease and use motivational skills. Apply problem solving/asset-based approaches to help people overcome barriers to pulmonary rehabilitation.
* Have a good grasp of the principles of pulmonary rehabilitation and be familiar with and able to discuss how to apply the British Thoracic Society guidelines for pulmonary rehabilitation.
* Complete assessment, care planning, education, interventions to support respiratory management & liaison with other healthcare professionals.
* Work as an autonomous practitioner on home visits; at weekends you will be a lone worker, with support available from team & division colleagues when needed.
* Current duties may include evenings as part of an expanded pulmonary rehabilitation offer; this will be planned via rosters.


Qualifications/Experience

* Experience in community or respiratory settings; band 6 or band 5 background.
* Driving licence & access to a car (essential).
* Skills in engaging with people, putting them at ease and maintaining motivation.
* Good grasp of pulmonary rehabilitation principles; familiar with British Thoracic Society guidelines.
* Flexible to rosters including potential evenings.
