Job Summary As a Community Care Officer, you will play a vital role in helping individuals achieve their personal goals and aspirations. Role Responsibilities As a Community Care Officer, your responsibilities will include: Identifying Goals: Work with clients to discover their personal goals and aspirations. Coordinating Care: Organise a range of outcomes to meet individual needs. Promoting Independence: Encourage clients to take control of their own care and support. Collaborating with Professionals: Work alongside other professionals to provide comprehensive care. Managing Caseloads: Effectively manage your caseload to ensure timely and quality support. Required Skills and Qualifications To excel in this role, you will need: A relevant qualification in social care or a related field. Experience in adult social care or a similar environment. Knowledge of strength-based approaches and person-centred care. Required Soft Skills or Competencies/Behaviours We are looking for candidates who demonstrate: Empathy and Compassion: A genuine desire to help others and understand their needs.
