About the Post
We are looking for a passionate and values, driven Social Worker to join our Adult Social Care team, working at the heart of our strategy for change to provide the right support, at the right time, in the right way.
You’ll help shape a system that prioritises prevention, strengths and community based solutions to improve outcomes for people.
You will lead on assessments, care planning and reviews using a strengths based, person centred approach, working alongside partners, families and communities to co‑produce solutions that build on what matters to people. You’ll support independence, manage risk proportionately and contribute to integrated, outcome-focused practice that reduces reliance on long-term services while ensuring people are safe and supported.
We are looking for a qualified Social Worker with strong professional curiosity, sound knowledge of legislation and a commitment to doing things differently.
Responsibilities
* Lead assessments, care planning and reviews in a strengths-based, person-centred perspective.
* Co‑produce solutions with partners, families and communities.
* Support independence and manage risk proportionately.
* Contribute to integrated, outcome-focused practice and reduce reliance on long-term services.
Qualifications and Experience
* Qualified Social Worker with relevant statutory work experience.
* Sound knowledge of relevant legislation and policy.
* Strong professional curiosity and commitment to innovative practice.

* Competitive salary within £35,412 – £46,142 (Career grade).
