Overview
This is a new and exciting collaboration between Dementia UK and 6 Primary Care Networks in Sheffield. Admiral Nurses are specialist dementia nurses who offer expert practical, clinical and emotional support to families involved in the care of a person living with dementia with complex needs. Based within multi-disciplinary/agency community teams and working alongside nurses, therapists, primary care, social care and third sector providers, you will also benefit from being a part of a Clinical Network of Admiral Nurses. The post holder will need to have the ability to work in a variety of settings. Dementia UK will support you to maintain and strengthen your clinical and leadership expertise as a specialist Admiral Nurse through regular clinical supervision and practice and professional development opportunities. Candidates are encouraged to complete the Pre-Admiral Nursing Module to prepare for becoming an Admiral Nurse. Details about this e-learning can be found on the Dementia UK website.
Main duties of the job include delivering and coordinating relationship-centred dementia care across Dovercourt Surgery and the 6 PCNs involved in the project, to improve the experience of carers/families affected by dementia with complex needs. The post holder will provide specialist level nursing support in line with the Admiral Nurse Competency framework.
Main duties & responsibilities
Clinical
* Advise on the care of people with dementia and their carers/families with complex care needs from peri-diagnosis to post-bereavement support
* Provide specialist nursing assessment, using the Admiral Nurse Assessment Framework for carers/families affected by dementia
* Provide a range of bio-psychosocial interventions to promote health and wellbeing for people with dementia and their carers/families
* Work collaboratively with other professionals to improve the identification and management of co-morbidities and frailty for people with dementia and their carers/families
* Develop and support care plans for people with dementia and families including positive risk management
* Support people with dementia, carers/families and staff to understand and respond to changes in behaviour and relationships as a result of dementia
* Provide care coordination for a clinical caseload of families with complex needs as identified within triage assessment in clinics or via home visit if required
* Ensure safeguarding of vulnerable adults and that all legal requirements relating to MCA 2005, MHA 1983 (amended 2007) and DoLS (2009) are considered and adhered to in patient care
* Act as an advocate for people with dementia and their carers/families to promote their rights and best interests
* Participate in case conferences and best interest meetings where required
* Work collaboratively with health, social and voluntary sector services to achieve coordinated care
* Promote advance care planning for people with dementia and support families around decisions and end-of-life care
* Manage care transitions and support people with dementia and their carers/families to reduce crisis situations
* Ensure care is delivered in accordance with local and national guidelines and host/PCN policies
* Recognise the limits of own competency and make appropriate referrals when required
Education & Support
* Facilitate and support the development and delivery of innovative dementia training and education to staff
* Support formal training events promoting best practice in end-of-life dementia care and disseminating Admiral Nursing work
* Provide mentorship and act as a role model on evidence-based dementia care within Dovercourt Surgery and the 6 PCNs
* Support development of dementia strategy work streams and policy development with local stakeholders
* Promote integrated care pathways and holistic models of care through dissemination of specialist skills
Evaluation and Audit
* Support audit and evaluation of quality improvements and initiatives to improve dementia care within the host and PCNs
* Collect quantitative and qualitative data to support evaluation and audit of the Admiral Nurse Service
* Ensure information and data recorded are relevant, accurate and contemporaneous
Professional
* Ensure practice complies with Dovercourt Surgery policies and the NMC Code
* Attend and participate in practice development, clinical supervision, and continuing professional development supported by Dementia UK
* Maintain professional registration in line with NMC guidance and re-validation
* Contribute to policy development relevant to dementia and Admiral Nursing within the host and PCNs
* Maintain up-to-date knowledge of evidence-based recommendations and foster positive working relationships with Dementia UK
Person Specification
Experience
* Post-registration clinical experience supporting people with dementia and their carers/families
* Experience of holistic nursing assessments, bio-psychosocial interventions, autonomous practice, and collaborative multi-agency working
Desirable
* Experience in a service-specific setting
* Experience in managing care transitions
* Experience of professional supervision/mentoring and facilitating training
Qualifications
* Registered Nurse (RN/RNMH/RNLD)
* Evidence of specialist training/education in dementia and ongoing CPD
* Commitment to continued professional development through Dementia UK’s Admiral Nurse Academy
* Desirable: Postgraduate qualification in a relevant area
