Main duties of the job
This role is part of our Rapid Personalised Care Service (RPCS),which delivers compassionate, end-of-life care to people in their own homesduring the final weeks of life. You do not need formal medical experience toapply just a caring nature, compassion, and a genuine desire to help andsupport others at the end of their lives. Full training and support will beprovided.
As a Health Care Assistant, you will:
Provide personalised care and support topatients, families and carers at home.
Usually work in pairs, with occasional loneworking, depending on the needs of the service.
Be supported remotely by the RPCS managementteam while working in the community.
Work closely with hospice colleagues andcommunity partners to deliver joined-up care.
Champion the Rennie Grove Peace philosophy inthe local community.
Develop your skills within the RPCS knowledgeand competency framework, with full support provided.
Drive between appointments, therefore a carand full driving licence are essential for this role.

We provide palliative and end oflife care support, including bereavement services, within
Buckinghamshire and Hertfordshire and our patient servicesare available to adults, as well as patients transitioning into adult services(16 to 25 years.) Bereavement support isoffered for all ages to families of patients and those around them, as well asthroughout the community, in schools and workplaces.
We are proud to offer care in the place that best suits theindividuals needs, whether thats at home, in a care home, in a short stay inour inpatient unit, out in the community, or in our purpose-built Living WellCentre facilities. As a hospice carecharity, we have offered this support, free of charge, for over 40 years.
Our staff team of 340+ includes a wide range ofpatient-facing disciplines plus operational support based in Watford, Tring, StAlbans, Chalfont St Giles and Berkhamsted. The Charity is now one of the largerUK hospice charities, reaching a GP population of 797,037 of whom 75% live inHertfordshire and 25% in Buckinghamshire.
Job responsibilities
Main Duties and Responsibilities
* Deliverconsistently high standards of individualised personal care and support underguidance of the RPCS Management team.
* Workalongside the patients families and/or health care professionals, maintainingan approach that incorporates physical, psychological, social and spiritualaspects of care in accordance with the Rennie Grove Peace philosophy.
* Observe and report changes in the patientssymptoms, conditions and circumstances to the RPCS Management and notify otherhealthcare professionals as necessary.
* Deliver care in accordance with the patientsagreed care plan.
* Identifyand act on any safeguarding concerns for patients or their families, in linewith Rennie Grove Peace policies. Keep accurate and legible records inaccordance with legislative requirements and Rennie Grove Peace practice andprocedure.
* Assist with the patients washing, bathing,showering, pressure area and oral care.
* Assist with the patients nutritionalrequirements including preparation of light meals and drinks according todietary requirements and choice.
* Assist with the patients toileting, which mayinclude the use of a bed pan, commode or lavatory.
* Prompt medication if dispensed in a pharmacyblister pack.
* Ensure all verbal and written communication isaccurate and timely.

* Care certificate/NVQ Level 2 or equivalent
* Basic Food Hygiene Certificate

* Experience of working within a community setting
* Knowledge of and experience in care at the end of life and after death
* Relevant experience in assisting patients/people with their personal hygiene
