Overview
We are looking for part-time Care Navigators to join our friendly team at Raj Medical Centre. This is a Fixed Term position to cover Maternity Leave.
Raj Medical Centre is a high achieving practice with 6800 registered patients. The Practice strives to meet the needs of the patient population and is always looking at new services and expertise to accomplish this.
Experience of working in General Practice and using SystmOne is desirable, however full training will be given.

2. Answering the Telephone
3. Booking appointments
4. Dealing with patient queries
5. Prescriptions
6. Scanning/data entry onto the clinical system

Job responsibilities
1. Telephone Duties
1. Care Navigation
2. Making Appointments
3. Giving out results
4. Taking Prescription requests
5. Contacting other providers
6. General enquiries
2. General Reception Duties
1. Greeting and directing patients
2. Handing out prescriptions
3. Ensuring the Task list is completed on a daily basis
4. Check NHS Mail account and print off patient correspondence
5. Inputting and extracting information from the practice computer system
6. Responding to the needs of both Doctors and Nurses during surgery
7. Health and Safety awareness (e.g handling samples)
8. General housekeeping e.g keeping the waiting areas and reception tidy
9. Collecting monies from patients for non NHS services
3. Repeat Prescriptions
1. Dealing with requests for repeat prescriptions
2. Raising prescriptions as per the practice protocols
3. Dealing with any queries relating to repeat prescriptions
4. Scanning/Read Coding
1. Scan all patient letters received and match to patients on a daily basis
2. Read code patient letters as per practice protocol on a daily basis
Person Specification
Qualifications
* Literacy and numeracy skills
* Understanding of the role of Care Navigator
* Skills and competency relevant to the role e.g. Communication, Telephone skills and Customer Service
* IT Literate
* Motivated and reliable
* Flexible and responsive to change
* Ability to work in a pressured environment
* Team Player
* Previous experience in General Practice
